In order to submit this form, please select the relevant code under Code Provided by Dolly Support:
For apartment moves: APT - 0001
For general COIs: GFM - 0001
Please note that we require a minimum of 24 business hours before your scheduled Dolly for processing or risk a delay in processing or a rejected COI request. Once we receive the requested COI details, processing will take approximately 24-48 hours. COIs are only processed during weekdays between 9am to 5pm CST. If a Dolly has been completed, we will be unable to provide a modified COI at that time.
