Any applicants that have their insurance under their business name, we require one extra document to verify insurance. In addition to insurance documents, we require documents that prove that you are covered under the business either as an owner or a driver.
Business ownership documents must:
- Include the name of the business, which must match the insurance documents. (e.g. if the business is named Acme LLC, the insurance should also cover Acme LLC.)
- Include your name, which must match the name you put on your application and on your license.
- Be issued from the government or reputable body (e.g. IRS, Better Business Bureau, State Government).
- Internal company memos cannot be used.

The above document is from the IRS, includes ACME LLC and has Theresa's name in the document.
The above document is from the State of Indiana and includes the business name (Fairuz Hoque LLC) in the name. However, it doesn't verify what his full name is on the application. This would not be accepted.
